Ashland Free Medical Clinic (“AFMC,” “we,” “us”), 16539 Ashland Avenue, San Lorenzo, CA 94580, offers a text messaging program for patients. If you opt in, we will send text messages related to your care at AFMC, such as appointment reminders and scheduling changes. We do not send marketing, promotional, or fundraising text messages.

Text messages are for appointment reminders and non-urgent communication only. Do not use text messages for medical emergencies. Call 911.
We keep messages brief and do not include detailed health information in them. Please be aware that text messages are not a fully secure form of communication, and anyone with access to your phone may see them.
Please tell us if you change your phone number or no longer use the number you gave us.
How to opt in
You opt in by completing the PATIENT COMMUNICATIONS & ELECTRONIC CONSENT form. By opting in, you agree to receive recurring automated text messages from AFMC at the mobile number you provide, and you agree to these terms.
Consent to receive text messages is not a condition of receiving care at AFMC.

Message frequency varies depending on your appointments and care needs.
Message and data rates
Message and data rates may apply. Charges depend on your mobile carrier and plan. AFMC does not charge for text messages.
How to opt out (STOP)
You can cancel the program at any time. Reply STOP to any message. You will receive one confirmation message, and then no more text messages will be sent to your number. If you want to receive messages again later, contact the clinic.
